My question to that would be why are they continuing to give this guy all kinds of access to medications? This country is currently going through a massive crisis over medication. Specifically painkillers. I know anti-depression medication isn't treated the same as painkillers because it's not (supposedly) being abused to the same degree, but you'd still think a free clinic would try to avoid over-prescribing medications to people.
I mean, ultimately, my question is about why any doctor would continue to prescribe anything to Spoony. I seriously doubt he has anything wrong with him except maybe some kind of sociopathic disorder like HPD. Spoony has spent years that we know of bouncing from treatment to treatment and apparently none of it has helped him at all. He's more unhinged now than he ever was. And he won't comply with any kind of mental health treatment or physical therapy. The only thing it sounds like he's willing to do is pop fucking pills.

The spiel with anti-depressants is that they're mostly harmless and aren't addictive while the real issue going on is with narcotics, notably opiates. And yes, family doctors can prescibe anti-depressants for the same reason. Alternately, no one buys anti-depressants on the street because... why? Last I checked, Percoset goes for about 30$ a pill.
Sexual issues aside (and that's why almost every doctor will try Welbutrin first as it's pretty much the only anti-depressant that can restore and/or not kill libido and sexual function) most of the rest of the side-effects are just acclimatization and the removal of what amounts to the patient's own brand of crazy propping them up. Because of how the FDA works, pretty much every side-effect ever listed by the study groups must be included.
Some dude hasn't actually fucking slept in three years due to some kind of mania? Finally sleeps properly for the first time and the brain doesn't know how to deal with downtime yet? Boom! Weird dreams get listed. And so on, and such. Though, yes, some of the last resort medications do cross over into the what-the-hell range.
