There are interviews with Fukushima where he literally explains what he did on the first two MGS games. He was just someone who helped out on the script, but Kojima was still the lead writer in all of his games.

No. Not by a long shot. I don't mean it as in the team does everything or the boss tells them what to do down to the second.
He can be genuinely brilliant. He just has to be wrangled very hard so he doesn't suffocate on his own farts. Some people just can't handle praise. Even mild praise that is well-earned will ruin their mojo immediately.

But you're right that his biggest problem is that he's a starfucker that finds it insufferable that what he is known for and good at isn't what he desires. He is terminally infected with the Hollywood disease.
It's bad enough with people that are a big fish in their small pond that don't realize they aren't shit in the big pond but want to swim in it regardless. But Kojima is a big fish in a big pond that is embarrassingly desperate to enter the smaller, grimy shit pond of Paedowood, which he perceives as more glamorous. It's very difficult to watch and the closer he gets the worse his creations get.

Except that's not how it was originally intended. In the Japanese release it's Clearly meant to be more of a comedic moment with both Miller and Cecile Frantically spouting this in a cult like fashion where snake just looks on like "WTF?"

English release makes it seem like kojima was just jerking himself off. I'm Not blaming the translator or voice actor just seems lost in translation

That doesn't mean he doesn't know anything about game design and that he can't make good games, but he often comes across as someone who is way too into his own ideas and full of himself.

He reminds me a bit of Monte Cook (a former D&D writer who is full of himself and is now making "revolutionary" games with ideas lifted from non-D&D games he has vaguely heard about).

It's a good thing most of them haven't played Policenauts, which allows the player to sexually harass almost every female NPC. Snatcher also had a lot of risque stuff that got toned down in the localized Sega CD port.
