Those people understand, on a theoretical level, that not going into work today because their guild is raiding will eventually fuck them over and start a chain of events that leads to homelessness and suicide, but they won't control their impulses anyway.

I believe you can get a mandatory psych hold if it's proven you're mentally incompetent to the point where you can't take care of your basic needs, the law varies state by state though. At the very least Miles has to have a full blown intervention with him and get him to agree to a hospital stay where he's forced to get on meds that actually help him and make actual progress before he can go home. I don't know what meds he's taking that he references on twitter, but they sure as shit ain't helping. Of course to go willingly he'd have to have at least a shred of self preservation left and he seems to actively defy self preservation.

I'm not so sure about that. The red dragon wasn't exactly a price-winning horse, despite the similar teeth, but the warthog is another quality of uggo fatty.
Plus she's not happy about Spoony's behaviour and has stated that multiple times. She'd want to change him right away. That surely could only be for the better but an old dog doesn't learn new tricks and his current behaviour is hard wired by now. Then again, it'd be a wonderful disaster.
