Adult medium should be for an average adult, not kid's size. That's literally what it means, "in the middle." Extra large shirts should be for extra large people. If you insist on calling your clothes made for a perfectly ordinary sized adult "extra large" and your clothes for manlets "large" you come across as a little sensitive about your size, in one way or another.
